Ramps can simply made of triangles, but you can make larger or more elaborate ones depending on your needs. Momentum and speed definitely help, of course, but you’ll be shocked the distances you can cover if you drive up to a ramp from a flat run up.Įxperimenting with ramp of differing angles will help with this - take your time, and place your girders precisely up against the grid. In Bridge Constructor Portal you’ll be able to fly a surprising distance with only a small ramp. In general though, cables are weightless additions that can support a decent amount of weight with few drawbacks, though they’re certainly not as capable as girders are.Ĭables are good for hanging girders and bridges in awkward positions, and if you’re skilled enough, you can even use them to swing platforms. ![]() Cables can be used as supports, and unlike girders, you can stretch them as long as you like, meaning you can hang a bridge in suspense, as long as you’re smart about weight distribution. ![]() This however just might get you around the weight issue. Overbuilding won’t be a big problem, but you definitely need to keep it in mind when trying to balance longer and larger structures. In some situations, a bridge with less support girders, to reduce the weight, might be beneficial, though that’s mainly in situations where you have both above and below supports. Overbuilding bridges can be a problem, especially if there are few connection points to distribute the weight to. Now I did say you can feel comfortable adding plenty of girders, but weight is an important factor. You do have to factor in stress points and uneven weight, but you can feel fairly comfortable adding a lot of support girders in the name of stability. You’ll come across many instances where you’ll have odd connection points and may struggle to get a bridge in place, but triangular supports along the top or bottom (or maybe both) will help bridges stay in place, even in odd positions. A straight bridge with just a couple of overhead connection points can hold steady if you have triangular girder shapes to support the bridge along its length. Triangle shapes will be your go-to method of strengthening your structures. You can use cables to hang your bridge on distant connection points too, or even arch your like a rounded doorway (yes, an archway) to enhance its strength. To start strengthening your bridges, you’ll need to form girders around your bridges to act as supports, connecting your bridge and distributing the weight to other connection points, if possible. ![]() Stretch girders and string them together to form basic bridges, and tap them to make them roads your vehicles can cross. I mean, the game is called Bridge Constructor Portal, so it’s no surprise you’ll be building bridges, but ensuring your structures are going to handle one vehicle, or even a whole convoy, is going to require work. Read through our advice and you’ll be well prepared for the road ahead, but if you’re in doubt, check the Best Practices page in the menu.įor more advice on thinking with Portals, check out our guide. You’ll be building a variety of bridges and ramps in Bridge Constructor Portal, so you’ll need to be aware of weights, supports, arches and more in order to get through these test chambers. So you’re already a Portal pro, huh? Well that’s good, but how are you with building bridges? If you’re not the best, then welcome to Bridge Building 101, where we’ll run you through how to get from A to B.

![]() ![]() This study was reviewed by the BIDMC Committee on Clinical Investigation and did not meet the definition of Human Subjects Research. No other significant changes occurred in the practice during this time period, and we made no change to staffing levels of medical assistants, nurses, telephone staff, or administrative assistants. Control physicians experienced no change to their visit lengths. Then, for the next 12 weeks, intervention physicians used scribes with their visit lengths shortened from 20 to 15 min and from 40 to 30 min, increasing the number of patient slots available in each 4-h session. We then implemented scribes in a transitional 3-week “honeymoon” period, in which scribes were provided to the four intervention physicians, but no change to standard visit lengths was made this was intended to represent the expected period of adaptation for patients, physicians, and staff. We first evaluated physician and patient outcomes for 4 weeks at baseline, prior to any modification in the practice. We tested the effect of scribes using a difference-in-differences approach, in which we compared differences between intervention and control periods among physicians who used scribes with the corresponding differences among physicians who did not use scribes during the identical periods. We focused on a real-world implementation model, in which provider workload was adjusted a priori to account for the increased costs incurred by scribes. Here, we present the detailed results of a single-center, quasi-experimental study of the effect of medical scribes on patient satisfaction, provider satisfaction, and provider productivity. 13, 14 Despite promising results, these studies often lack generalizability and fail to provide crucial information about the effects of scribes on patient and provider satisfaction or how to accommodate the increased costs of employing scribes. Collectively, these demonstrate benefit of medical scribes to providers, patients, and healthcare systems. 10– 12 As recently as 2014, a meta-analysis concluded that “confidence in the reliability of the evidence is significantly constrained,” and called for “methodologically and sufficiently powered studies.” Since then, several studies examining the use of medical scribes in the primary care setting have been published. Until recently, evidence on the benefits and tradeoffs of medical scribes has been limited to the emergency and ambulatory subspecialty practice settings. One recent editorial by the American College of Medical Scribe Specialties estimated “industry ranks to swell to 100,000 by 2020.” 8Ī review of existing literature reflects this trend. 9 Scribes have been employed in emergency departments since the late twentieth century 7 and their use has grown steadily since. ![]() Duties include real-time transcribing of physician notes, organization of objective healthcare data, point-of-service collections, and patient tracking. 6– 8Ī medical scribe is a non-clinical ancillary staff member who assists a licensed-independent provider. 2– 5 A number of solutions to this largely uncompensated effort have been proposed, ranging from improving the usability of the EHR to medical scribes. Outside of office hours, physicians spend another 1 to 2 h of personal time daily performing additional computer and clerical work. ![]() 1 For every hour that physicians provide direct clinical face time to patients, two additional hours are spent on EHR and desk work within the clinic day. This dynamic has a measurable negative effect on patient centeredness and physician work-life balance. With increasing use of the medical record as an instrument for billing, and particularly with advent of the electronic health record (EHR), the medical note may seem as important as the actual patient care. Documentation, once a small part of the patient encounter, has grown to become a major driver of physician time. ![]() The organization noted that any lack of standardization placed patients at increased risk for medical errors and serious adverse events.īecause the greatest number of handoff opportunities occur during shift-to-shift transitions, Nebraska Medicine chose to focus its initial efforts on improving the process at this level. Although the organization used the SBAR (Situation, Background, Assessment, Recommendation) tool for handoffs and encouraged patient and family participation in the process, an opportunity for improvement and reduced variability existed. Nebraska Medicine, an academic medical center in Omaha, recognized the variability in its handoff process across units and disciplines. In addition, handoffs conducted at the bedside encourage the patient and family to participate in the care plan. ![]() Handoffs completed at the patient’s bedside-which allow for direct patient visualization and communication between caregivers-improve the process. According to Wheeler, approximately 70% of serious medical errors are the result of ineffective handoff communication. When handoff is compromised, the patient is placed at an increased risk for an adverse event. Most communication errors occur during this information transfer, especially during shift-to-shift handoff.
